If you have been on TikTok lately, you may have seen it: a plastic surgeon performing a facelift and finding filler that was injected years earlier.
Which raises a pretty obvious question. Wasn't that filler supposed to be gone by now?
Hyaluronic acid fillers are considered temporary, and patients are usually given an expected timeline for how long their results will last. But research using MRI and other imaging has shown that filler can sometimes remain detectable years after it was injected.
That does not mean filler is bad, or that everyone who has ever had filler still has it sitting in their face. It does mean that filler can sometimes last much longer than initially expected.

So, does filler really dissolve? And what can happen to it years after it was injected?
Does Filler Really Dissolve?
Yes, hyaluronic acid filler does break down over time. The part that has changed is our understanding of how long that process can take.
Hyaluronic acid is naturally broken down by the body, which is why HA fillers are considered temporary. However, newer imaging has shown that some filler can remain detectable much longer than initially expected, sometimes years after it was injected.
How long filler remains can depend on the product, where it was injected and your individual body. This also means that seeing your filler results fade does not necessarily mean all of the filler is gone.
What Happens to Filler Over Time?
After hyaluronic acid filler is injected, the body gradually begins to break it down. Over time, the visible effect becomes less noticeable as the amount of filler decreases.
What we now know is that this can take much longer than initially expected. Even when you can no longer see the same volume or fullness, some filler may still remain in the tissues.
Filler can also migrate, meaning it moves beyond the area where it was originally placed. However, filler that is still present years later has not necessarily migrated. It may simply have lasted longer than expected.
So, when your filler looks like it is “gone,” that does not necessarily mean there is no filler left.
Filler Fading vs. Filler Remaining vs. Filler Migration
Filler fading, remaining and migrating are often talked about as though they mean the same thing. They don't.
This distinction matters when deciding whether you actually need more filler. If the original result has faded but filler is still present, adding more may simply create more volume rather than the result you are looking for.
Can Repeated Filler Lead to an Overfilled Look?
It can. If some of your previous filler is still present, adding more every time the visible result starts to fade can gradually create more volume than intended.
This does not mean that getting filler regularly will automatically make you look overfilled. The amount used, where it is placed and what is already present all matter.
Sometimes more filler is the right choice. Other times, it may make more sense to dissolve old filler, change the treatment plan or simply leave things alone. At art&fact, our providers assess what is already there and create a customized treatment plan based on what you actually need.
Do You Actually Need More Filler?
Not necessarily. Filler should not automatically be topped up simply because a certain amount of time has passed.

Can you dissolve filler years after it was injected?
Yes. Hyaluronic acid filler can be dissolved with hyaluronidase, even years after it was injected. Whether it actually needs to be dissolved is a different question and depends on what your injector finds during your assessment.
How do I know if I still have old filler?
You may not be able to tell just by looking at your face. Your treatment history and an in-person assessment can help your injector determine what may still be there. In some cases, ultrasound can also be used to locate existing filler.
Should I tell my injector about filler I had somewhere else?
Yes. Tell your injector where you have previously had filler, approximately when you had it and which product was used, if you know. Even filler from several years ago can be relevant when planning your next treatment.
Do you need to dissolve filler before a facelift?
Not necessarily. Having previous filler does not automatically mean it needs to be dissolved before surgery. Your surgeon will assess what you have had done and whether it changes your facelift plan.
